A New System for Paperwork in 2021...
The Utica High School Athletic Department will now be transitioning to all online pre-participation athletic forms starting for the winter 20-21 season. Parents and students are encouraged to create your accounts now and complete the necessary ONE-TIME forms that will allow you to be eligible to tryout/join any sport for any season throughout this school year. Yes, you heard that, ONE TIME. So, for those multisport athletes who have had to fill out the same forms for each season, will no longer have to do that. You can upload new physicals and/or complete this year’s one-time MHSAA health waiver form as well. Spring sport parents and athletes are encouraged to do this now as well, why wait!
Key Points to Remember:
1. The parent and the athlete MUST have separate accounts using different emails and/or cell numbers.
2. Parents must remember to LINK to their students account once that is created. This allows for the parent and the student to be able to complete the signature’s required on all the forms.
3. Lacrosse families will not utilize the online payment through PaySchool. Since we are a club, we handle all our own finances and it does not go through the school district.

2020 Informational Meeting Paperwork
Because we are a club sport, we operate by two simple rules: no one can practice until all required forms are turned in and no one receives a game jersey until their registration is paid in full. Our board of directors are volunteers and these are designed to help make their jobs easier.
These five forms must be on file with the club before any player begins practicing with the team. The only exceptions are the medical history and physical forms that may already be on file with the athletic director. We will make sure we get those from the office.
This is the complete packet that includes all paperwork for the start of the season. If you need individual forms, you can click below. Please remember that your child's medical history and physical form may already be on file with the athletic director and we will get those forms from the office.
Return To Activity Form
This is the one form that we hope no one has to use this season. It is the Return To Activity - Post-Concussion Consent Form. MHSAA rules now require that on top of clearance from a physician after a player sustains a concussion, that a physician, PA or nurse practitioner sign this form as well, so the school can keep it on file.
